Kaiser Permanente has partnered with a network of external providers for alternative options to help you get the care you need. To use these external providers for routine eye exams, contact lenses and eyewear needs, please review the following information:

What is covered:

• Services-Routine eye exam for glasses, contacts, and initial contact lens exams.
• Benefits-Your Kaiser Permanente benefits will be the same using an eternal provider with exceptions listed below.

Exceptions:

• Members utilizing the Medical Financial Assistance (MFA) program, are not covered with external Kaiser Permanente Partners:
  Medical Financial Assistance (MFA)
  Oregon Health Plan (OHP)
  Washington Medicaid


• Contact Lens Fitting Fee:
  Members seen by an external Kaiser Permanente Partner for a contact lens exam will be subject to these partners contact lens fitting fee. These fees apply to children and adults.
  
  Medically Necessary Eye Care: Members with any symptoms or needing a medical eye exam (diabetic eye exam, cataract check, etc.) will need to be seen at Kaiser Permanente.
  
  Hardware: Members can get prescription glasses and contact lenses at an external Kaiser Permanente Partner location. If a member chooses to purchase hardware at an external Kaiser Permanente Partner location, members will pay out of pocket and submit for reimbursement at Kaiser.
  
  • How to get reimbursed: Fill out the reimbursement form on kp.org by following these steps:
    Sign into kp.org -> Go to “Coverage & Costs” – then under “Understand your costs” click on ‘Submit a claim’.
    There, you may enter a claim directly online, or download a PDF to print, complete and mail.

  If an external provider informed a member they need cataract surgery, please phone Kaiser Membership Services 1-800-813-2000 option 3 and provide this information to an eye care representative.
