This Privacy Policy describes the types of information Kaiser Permanente Eye Care Services collects from you when you visit our Web site or order products from us, and explains how we use that information. Information collection and use
The kp.org/2020 Web site is owned and managed by Kaiser Permanente Eye Care Services, a department of the Kaiser Permanente medical care program.. Kaiser Permanente consists of Kaiser Foundation Hospitals and Kaiser Foundation Health Plan, Inc. (both nonprofit, public benefit corporations), and the Permanente Medical Groups (for-profit corporations and partnerships). Kaiser Permanente is America's largest nonprofit health plan, serving more than eight million members in nine states and the District of Columbia. The site is hosted and operated for Kaiser Permanente Eye Care Services by Jobson Medical Information, LLC, under a contract with The Permanente Medical Group.
We collect personally identifiable information from our members at different points on our Web site, as described below.
Kaiser Permanente members in Hawaii and Northern California may purchase contact lens refills on our site. If you purchase contact lens refills from us, we request certain “Protected Health Information” (PHI) from you on our order form. You must provide contact information (including, but not limited to, name, email, and shipping address) and financial information (including, but not limited to, credit card number and expiration date). We use this information for billing purposes and to fill your orders. If we have trouble processing an order, we will use this information to contact you.
Customer Service Surveys
Kaiser Permanente Eye Care Services values your feedback about our services, and we offer you an opportunity to take an online survey to rate your experiences with us. The survey is confidential and includes optional questions related to our Web site users’ demographic information, such as gender, race, and age. We use this information to monitor the interests and attitudes of our members, and to improve our services and products so they are relevant and helpful to our members.
Participation in this survey is voluntary and you have a choice whether or not to disclose any demographic information. Your decision on whether to submit a survey will not affect the health care services or health plan benefits you receive from Kaiser Permanente.
Log Files
We gather certain information automatically from our Web site users and store it in log files. This information includes internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data.
We use this information, which does not identify individual members and visitors, to analyze trends, to administer the site and to track members and visitors’ movements around the site. We do not link this automatically-collected data to PHI.
We use a third-party service under contract with Jobson Medical Information to track non-personally identifiable information about visitors to our site in the aggregate. Any information we track in this manner is anonymous and limited to usage and volume statistics. We use this information to help us design the site, identify popular features and market our services, and make the site more useful to visitors.
Cookies
A cookie is a small text file that is stored on a member or site visitor’s computer for record-keeping purposes. We may link the information we store in cookies to your personal information in order to provide a faster and more pleasant online experience.
We use “session ID” cookies to make it easier for you to navigate our site. We set a session cookie to store your password, so you don’t have to enter it more than once per session. A session ID cookie expires when you close your Web browser.

Customer Service
Based upon the personally identifiable information you provide us, we will communicate with you in response to your inquiries, to provide the services you request, and to manage your account. We will communicate with you by email or telephone, in accordance with the preferences you indicate to us.
Profile
We store information that we collect through cookies and your history of online activity on our site to create a “profile” of your preferences. We tie your personally identifiable information and your purchasing history to information in the profile, in order to provide tailored promotions and marketing offers, and to improve the content of the site for you. We do not share your profile with third parties except as otherwise stated in this Privacy Policy.
Supplementation of Information
Kaiser Permanente members in Hawaii and Northern California can use our online contact lens refill service. In order to provide secure and convenient online ordering, we contract with a third party vendor, Verisign Inc., to verify your credit card information and billing address.

Aggregate Information (non-personally identifiable)
We may share aggregated demographic information about Web site users with third parties under contract with Kaiser Permanente, including agencies and third party vendors, in order to obtain aggregate demographic information about our Web site users marketing analysis and assistance with our marketing strategies. This information does not identify individual members or visitors to this Web site. We do not link aggregate user data with personally identifiable information.
Personally identifiable information shared with Third Parties:
When you order a product from this site we share your personally identifiable information with our contact lens fulfillment vendor. We use an outside shipping company to fulfill orders, and a credit card processing to bill you for goods and services. These companies do not retain, share, store or use personally identifiable information for any other purposes. Other than these companies and our Web site operator identified above, we do not share your personally identifiable information with any other third parties.
Additional Disclosures
We reserve the right to disclose your personally identifiable information as required by law or when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process served on our Web site.
Choice and Opt-out
We provide you the opportunity to ‘opt-out’ of having your personally identifiable information used for certain purposes, or if you no longer wish to receive our promotional communications. You can indicate your preference on our email communications by simply checking the box provided to unsubscribe.

No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore, while we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security.
